Throughout the COVID-19 Pandemic, the Surgery has remained open and provided additional capacity to our Patients through new digital consulting solutions.

We also opened over the Bank Holiday Weekends.

We are now starting to reflect on the past 8 months and wanted to share some information with our Patients. Data has been gathered between March 1st to October 16th 2020 for 4038 Patients registered with Roe Lane Surgery.

How to register as a Patient at Roe Lane Surgery

Roe Lane Surgery has recently reduced its Practice Boundary and we can only accept patients who are within our boundary area.

If you live in our practice catchment area you are welcome to join Roe Lane Surgery as a patient.

To register you must complete a GMS1 Form, New Patient Registration Form, and our Alcohol Audit questionnaire These 3 forms MUST be downloaded and printed from the website or alternatively please pop into the Practice and request paper copies from a member of the reception team.

Please take the time to read this leaflet regarding alcohol and drink awareness which coincides with the Alcohol audit questionnaire.

We will ask you to also provide:

  • Proof of photographic ID (e.g. passport or driving licence [you may be asked for two forms of ID)
  • Proof of Address (e.g. utility bill or bank/credit card statement from the last four months, current tenancy agreement, government agency letter, higher education letter or a letter from a recognised employer).

When registering a child who is under 16 years of age, a separate New Patient Registration form will be provided to you.

**Parents will be required to bring in a copy of the child’s birth certificate when registering, and proof of address if online access is requested for patients aged 12 and under**

For new baby registrations, you are welcome to complete a GMS1 form (please ensure that you provide an NHS number on the completed form). Please pop into the Practice or download the GMS1 form here
